The CORE Initiative

The CORE Initiative addresses the opioid crisis in our community through three interconnected pillars: saving lives in the moment, supporting recovery after the crisis, and removing the financial barriers that stand between people and healing.

KEY “CORE” SERVICES

PORT (Post Overdose Response Team)

Rapid, trauma-informed follow-up after an overdose. Our team (counselor, medical staff, and peer specialist) provides support, connects individuals and families to recovery services, and helps remove barriers to care.

TRANSPORTATION:

Rides or gas vouchers to treatment, counseling, medical, court, and recovery-related appointments.

LONG-TERM CASE MANAGEMENT:

Ongoing check-ins, connection to treatment and support groups, treatment related financial assistance, and access to CHP clinic services.

NARCAN DISTRIBUTION and EDUCATION:

Naloxone kits and brief education provided to individuals, families, and community partners to help prevent overdose deaths.
